Post-hospital care is short-term support that helps you settle back at home and rebuild your strength after a hospital stay, operation, illness or injury. It can include practical help with washing, dressing and moving around safely, reminders for medication and follow-up appointments, meal preparation and gentle encouragement as your confidence returns. The aim is to make those first weeks at home feel calmer and more manageable, with support that eases as you recover.

How soon after discharge can care start?
